Reporting Accidents
Following an accident during a game, please be sure to complete an Accident Report Form at the ground you
are playing at and submit this through Ground Control. If you do for some reason not complete the form,
it may be accepted a week later, but please be sure the fully completed form is submitted through Ground
Control at Lilli Pilli.

Football NSW Policy
For further information about Insurance and the general programme in place for all football players in NSW,
please refer to the following page which has a detailed policy document you can download application
forms for claiming on the "Accident Support Program" policy.
The Accident Support Program operates Australia wide and does NOT afford any cover for members whilst engaged in footballing activities overseas.
Cover is provided for injuries caused by active participation in officially organised games, official practice and training sessions, and official functions.
The Accident Support Program is designed to supplement loss of income and non-Medicare expenses and is part of the Football NSW Limited Risk Management strategy.
